Throughout this agreement: "you", "your" and "we" refers to "Port Jervis Pet Sitting". "I", "me" and "my" refers to the pet owner. 
1. I give you permission to act as the guardian of my pets. 
2. I give you permission to take any appropriate action to keep my pets in good health. 
3. I give you permission to administer the stated medication to the named pet(s) for the duration of my absence. 
4. I give you permission to take photographs and videos of my pets and to use those pictures on your social media pages and website. To opt out email fosterandadoptpets@gmail.com.
5. I agree that it is safer to keep pets indoors during my absence. In the event my pet goes missing, I understand that you will continue to provide care visits for my pet(s) until I return. I authorise you to notify my emergency contact, my vet and the relevant authorities that my pet is missing from home. 
6. I agree that you have the right to refuse service for any reason without explanation. 
7. I agree that you cannot be held liable for the loss of my pet/s if they have access to an unlocked cat flap. 
8. I agree to inform you of any changes to my or my pets details, including changes to my pets health. 
9. I agree to inform you of any aggressive or badly behaved pets. I agree to be truthful in my pets behavior 
10. I agree to pay you the full cost of providing the service. 
11. I agree to pay you the full cost of calling out a locksmith if we are unable to access your property due to problems your locks or keys. 
12. I confirm that the details in Pet Owner Dashboard are correct. I understand you are not a grooming service and you will do your best to keep my pets clean
13. I confirm that I am responsible for all costs incurred, either veterinary or other, as a result of any sickness, accident or damage caused to or by my pet(s), excepting third-party liability, and that I will pay any such costs or expenses on demand. 
14. I accept that you will have no liability if a third party shares access to my property or pets whilst I am away (including but not limited to: friends, family, neighbors, cleaners, painters, builders etc.) 
15. I accept that no liability will attach to you. 
16. I agree this authorization is valid for this and any future bookings. 
17. I am aware there is a dog respitory illness going around the country. I agree to notify Diane if my dog presents any symptoms or has been in contact with a  dog with symptoms or illness. Diane and Port Jervis Pet Sitting has no liability if my dog contracts illness 
18. I am aware my pet may get dirty from playing and may get a bath if needed. I am  aware Diane is not a groomer or responsible if my pet gets dirty or breaks a nail, or long hair gets messy. I am responsible for making grooming appointments and arranging Diane to take my pet to get groomed when needed. I authorize Diane to bath and groom ( brush and trim) hair if it becomes a hazard . if Diane cannot reach me, she is authorized to bathe and trim my pets hair as needed to keep pet free from pain or excessive dirtiness. If my dog requires daily grooming i will make that clear prior to stay and provide written instructions we both agree on .
19. I am verifying my pet is spayed / or neutered. I am aware Diane does not take unfixed animals to her home.
20. I am aware if my pet starts to pee and mark in Diane's house she will use a belly band diaper.
21. I will contact Diane with any questions or concerns and will not hold Diane liable for my pets misbehavior 
22. I agree to all of these terms set forth 
23. If my pet refuses to take medication, Diane will notify me 
Charges and Fees: Refer to our websiteDiane does not take personal checks .
Public holidays: We reserve the right to charge an additional fee on public holidays. Refer to our website for full details.
Cancellation policy: We reserve the right to charge you if you cancel the booking. We will fully refund you if we cancel the booking. Refer to our website for full details. 
Personal information: We will only use your personal information to provide you with our pet care services. We will not pass your details onto any third parties. Please make sure your pets are up to date on all vaccines and flea and tick preventative 
A non refundable  deposit of $50 has to be made to secure your booking
